I know there were some very very early promo shots for DW where Matt Smith had a black hipster-ish outfit, and I’m wondering if this is the same deal: Did Karen ever actually wear this outfit on the show?

No she didn’t. In s6 she wears some plaid + jean combos, but she wears nothing even CLOSE to this in s5.
